At Our Lady of Mount. Carmel Parish we all work together to protect our children.
Below you will find the necessary information and documents needed to familiarize yourself with the process of the Diocesan Safe Environment requirements, how we protect the faithful, and the steps involved to minister, work, or volunteer in the Diocese.
All persons age 18 years old and older must be approved by the Office of Safe Environment (OSE) prior to ministering, working, or volunteering in the diocese. The approval process begins by completing the Essential Three Requirements as outlined in the Diocesan Policy for the Protection of the Faithful.

If you are a first time volunteer and over the age of 18 years old or if you are currently a volunteer that needs to update a specific form, follow the links below.

1. CORI form CORI forms, whether new or renewing, must be accompanied with a copy of your license, front & back (Must be renewed annually)
2. Code of Conduct form (Renewed only if the bishop makes updates to the form)
3. Safe Environment Videos (Training must be renewed every 6 years)
4. COPY of your driver's license, FRONT and BACK.
ATTENTION RI Residents
Are you a resident of RI? It is required by the Diocese of Fall River that a RI Resident - Authorization to Release Information be signed, notarized, and submitted together with the other forms. Click on the RI Resident - Authorization to Release Information link above located in our DOCUMENT section.
The Essential Three(s) MUST be completed within 15 business days. Failure to complete ALL the required documents, within the 15 days, will result in having to resubmit ALL paperwork with current dates.
If you need help with a computer or help with printing documents the parish office has a computer and a printer available to use. Please contact the office to arrange a time to come in.
Before submitting your forms, please review the Document Checklist above to make sure everything is completed. If you have ALL the forms required of the Essential Three(s), kindly return all your paperwork to the parish office. Please DO NOT submit any paperwork until ALL form(s) required are completed.
